YOUR DJ AND/OR KARAOKE HOST (from this point on, the term “DJ” will mean either “Disc Jockey” and/or “Karaoke Host” )  WILL BE:

Terms of Agreement

1. ½ of agreed balance required to bind terms.  Check should be payable to:  Gaylyn Shay
2. Any deposit received is non-refundable.
3. Parties entering into this contract are permanently bound to fulfill the obligations by this agreement unless one or more of the following conditions apply:
1. Both parties mutually agree to leave the agreement and its responsibilities.
2. The event described herein is completely cancelled, and a minimum of 30 days notice is given.
4. In the event that ABOVE AND BEYOND PRODUCTIONS becomes unable to provide  services due to complications beyond their control, a reputable replacement shall be provided at no more than the original price agreed on.
5. The Venue: ABOVE AND BEYOND PRODUCTIONS will always arrive prior to the agreed upon start time to accomplish equipment setup and configuration. Most of the time, ABOVE AND BEYOND PRODUCTIONS will have our professional equipment up and running in 30-45 minutes or less. While ABOVE AND BEYOND PRODUCTIONS will work with the venue contact (to be provided by the client) to ensure a suitable working area, the Client is responsible for providing ABOVE AND BEYOND PRODUCTIONS with a 'reasonably' secure stage or floor area with easy (handicap accessible ramp or elevator) access. The Client is required to ensure that the DJ operating location at the engagement venue is located within fifty (50) cable-feet of an AC outlet. If lights are included in the provided service, two independent 20 Amp outlets will be required to support the electrical current requirements of the equipment. If during the event the AC receptacles prove to be inadequate in supporting both the music/sound equipment and lights, the lights will be disconnected. The Client will NOT be entitled to a refund, nor will ABOVE AND BEYOND PRODUCTIONS be liable to refund any fees received or owed for temporary interruption to services due to inadequate AC power. ABOVE AND BEYOND PRODUCTIONS requires an approximant 8 x 8 foot area for the equipment, as well as a 3 x 3-foot area for both of the speaker tripods on each side of the dance floor. If lights are requested as part of this contract, tripod areas must be 5 x 5-feet in size. Smoke machines are not used because it sets off the smoke alarm systems.    IT IS STRESSED NOT TO WORRY ABOUT EXACT MEASUREMENTS AS LISTED ABOVE.  THESE ARE ONLY SUGGESTED MEASURED SPACES.

6. Any and all information or details must be communicated before the 5 days preceding the performance.
7. PERSONAL DAMAGES. Purchaser shall be held liable for any injury or damages to the DJ (person) or property of the DJ Service while on the premises of the said engagement if damage is caused by Purchaser, his guest(s), associates, engagement invitees, employees, or any other party in attendance, whether invited or not.
8. The parties purchasing the services of ABOVE AND BEYOND PRODUCTIONS agree to allow ABOVE AND BEYOND PRODUCTIONS to use pictures and video images taken at this event for promotional purposes.
9. ADEQUATE TABLE.  Purchaser must provide an adequate table for DJ’s equipment.
10. TRAVEL. Events requiring travel outside our primary service area of a 150 mile radius of Albany, Oregon will require accommodations for overnight stay in a local hotel/motel for DJ Service at Purchaser’s expense. Accommodations must be pre-approved by DJ Service. (*Only if initialed by both parties prior to signing). (DJ Service initials represent approval of hotel)
